Qualified candidates will have the skills, training, and experience to: Engage in and carry out an individualized program of physical therapy including assessment, treatment, planning, implementation, education, and communication to maximize a patient’s progress toward achieving functional goals; provide direct patient care, evaluate functional needs and outcomes, and consult with other specialists; review functional needs of patients and adjust care plans to ensure optimal patient outcomes; Design and implement a plan of care for patients based on a thorough assessment of potential development and incorporate participation by significant others whenever it seems appropriate and continuously conduct therapy practice within defined standards of care; Direct the assistance of support staff to achieve patient’s functional goals; communicate effectively with other professional and support staff to achieve positive patient outcomes; document evaluations, treatment goals, and plans; regularly update documentation and maintain prescriptions and all required signatures. Additional duties will include those within the realm of the PT field of knowledge and ability. Formal education that includes a degree in Physical Therapy with a minimum of one year of experience required. Qualified candidates will have a current, unencumbered license in Physical Therapy, excellent patient and co-worker communication; organizational, time management, and computer/EMR skills a must.
